Home Care Nursing - Island Health Community Health Services

Support and nursing care provided for patients in their home or community where applicable.
Island Health Community Health Services provides non-emergency nursing care at home or in the community for people who need support with health concerns such as:
  • Wound assessment and care e.g., skin tears due to fragility, diabetic foot ulcers and lower leg wounds, post-surgical wound, pressure ulcers, ostomy, and nephrostomy care
  • Palliative care
  • Post-hospital recovery
  • Chronic disease management
  • Medication and pain management
  • IV care including daily IV antibiotic therapy, IV therapy for extended antibiotic treatment in partnership with medical day care clinics

Eligibility
  • Patient must be a Canadian citizen, or have permanent resident status
  • Patient must be a resident of British Columbia
  • Patient requires care at home (after a hospital stay, instead of going to hospital, or because of a serious or terminal illness) and/or be unable to function independently due to chronic or end-stage illness
How to Access
  • Access starts through the Community Access Centres using a central intake and assessment process. During this process, clinicians will assess eligibility, clinical need, function, and safety to determine if Home Care Nursing is the right service. Services vary by location.
  • General public can call their local Community Access Centre(see below) to request further information and self-refer.
